- Please print this sheet, and closely follow each step of the application process.
- Typical rotation start date is 6 to 8 weeks after all documents and payments have been received.
- We must receive your payments and 'original' documents at least 30 days in advance of your rotation start date.
- You may fax a legible copy of your documents to U.S. +770 692 4957 and have your payments processed via credit card over the phone in order for us to hold your seat for a temporary period of time, but we must still receive your 'original' documents 4 weeks in advance of your requested rotation start date.
- Without payments, neither your faxed nor original documents will be processed.
- The original and faxed copies of your documents must be legible.

- Additional fees may apply.
- We accept checks, money orders and all major credit cards. Please make checks payable to "The Greater Medical Center".
- A $75 fee applies if you change your schedule less than 31 days before your rotation start date.
- A $150 fee applies if you change from one CMP program to another. Additionally, you will be responsible for the new tuition fee for the program you choose to enter.
